Relodipine which is a calcium channel blocker is a first choice for initial therapy in hypertensive aged 55 years or over or in black patients of any age according to the new NICE recommendation
Clinical Pharmacology
After oral administration of a therapeutic dose, RELODIPIN® is well absorbed with peak blood levels taking place between 6-12 hours postdose. Absolute bioavailability has been estimated to be between 64 and 80%. The volume of distribution is approximately 21L/kg. Steady-state plasma levels are reached after 7-8 days of consecutive dosing. Absorption or RELODIPIN® is unaffected by consumption of food. The terminal plasma elimination half-life is about 35-50 hours and is consistent with once daily dosing.
RELODIPIN® is extensively metabolized by the liver to inactive metabolites with 10% of the parent compound and 60% of metabolites excreted in the urine. Hemodynamic studies and exercise based controlled clinical trials in NHYA class 111-1V heart failure patients have shown that RELODIPINE® did not lead to clinical deterioration as measured by exercise tolerance, left ventricular ejection fraction and clinical symptomatology.
A place controlled study (PRAISE) designed to evacuate patients in NHYA class 111-1V heart failure receiving digoxin, diuretics and ACE inhibitors has shown that RELODIPIN® did not lead to an increase in the risk of mortality or combined mortality and morbidity in patients with heart failure.

RELODIPIN® has been safely administered with the thiazide diuretics, alpha-blockers, beta-blockers, angiotensin-converting enzyme inhibitors, long-acting nitrates, sublingual nitroglycerine, non-steroidal anti-inflammatory drugs, antibiotics and oral hypoglycemic drugs. Special studies have indicated that the co-administration of RELODIPIN® did not lead to increase in the risk of mortality or combined mortality and morbidity in patients with heart failure.
